7Medical Release & Liability Waiver

In consideration of the participant named in this form (the “Participant”) being permitted to participate in the Peak Team Building Camp (the “Camp”) hosted by Peak Soccer Club at Snow College, I, the parent or legal guardian, acknowledge and agree to the following:

1. Assumption of Risk

I understand that participation in soccer and overnight camp activities involves inherent risks, including the risk of serious injury. I knowingly and voluntarily accept these risks on behalf of the Participant.

2. Release of Liability

I release and hold harmless Peak Soccer Club, its coaches, staff, volunteers, and Snow College from any and all claims, demands, or causes of action arising from the Participant's involvement in the Camp, except those resulting from gross negligence or willful misconduct.

3. Medical Authorization

If the Participant requires medical attention and I cannot be reached, I authorize Camp staff to seek and consent to emergency medical treatment on my behalf. I accept responsibility for any resulting medical costs.

4. Allergies & Medical Conditions

I confirm that I have disclosed all known allergies, dietary restrictions, medications, and medical conditions in this registration.

5. Media Release

I grant Peak Soccer Club permission to use photographs and video of the Participant taken during the Camp for promotional purposes, without compensation.

6. Code of Conduct

I agree that the Participant will follow all Camp rules and the reasonable directions of Camp staff, and that Peak Soccer Club may dismiss a participant for serious misconduct without refund.
